Dos2 mordus
In Mordus' [1] basement you can find a head hanging on a wall. Interact with it. You notice that it lacks one eye. You can find it in The Wreckers Cave [2]. In the chamber, adjacent to the one where you met with Mordus, you can find a hatch. You can open it by using Mordus's Amulet. There, you find a body with the missing eye.I found it in Act 2 - Driftwood, in Mordus's basement after freeing the lich (behind the underground puzzle door) - can't seem to find any chest which uses it. There's an Acid-Stained Chest there. You may have already lockpicked it open.

Go to Mordus safehouse, acquire "Letter to Mordus" from cellar/underground cave. Keep this letter in invertory, don't talk to Lohar or Julian for now. In the area where you fight Mordus, abit further in there is a hatch that you can go down (near 2 dwarven ghosts). You'll find the missing "eye" down the hatch.
